Why aren't fields on the jotform appearing in the submitted data?

    We created a form (ID 2731221395) with several fields on it. When looking at the submissions, some of the fields on the form are absent. Please refer to the attached screenshot, For example, you'll see "Full Name" and "E-mail," which appear on both the form and the submission data. However, "Confirm E-mail," "Address," "Daytime Phone," and a couple of other fields are present on the form and absent from the submission information.

    Are we missing something? Is our form misconfigured? Are we bleeding data? Thanks for your help.
