I checked your form and tried accessing it in an android mobile device and I was not able to replicate the issue.
To further help, can you provide us what browser version and device you are using when you encountered the problem?

My browser is google. I have an android version 4.4.4 moto g.

I cloned your form and I can fill up your form using google chrome browser in an android device.
Can you try accessing my cloned form and see if it works in your mobile device? Here's the link to the cloned form: http://form.jotformpro.com/form/51448577731968. If it works, you may re-clone it back to you.
Please also try to clear your browser cache and form cache. Do let us know if issue persists.

To change your link you need to re-embed your form with the secure form option unchecked
Also embed using the iframe method.
