I inspected the form and the Spreadsheet didn't have any default values (formula) added:
To add a formula to this widget please select the wizard and then add the formula to the "default values" area as seen bellow:
Formula
d1 = b1*c1
d2 = b2*c2
d3 = b3*c3
d4 = b4*c4
d5 = b5*c5
d6 = b6*c6
d7 = b7*c7
d8 = b8*c8
d9 = b9*c9
d10 = b10*c10
d11 = b11*c11
d12 = b12*c12
d13 = d1+d2+d3+d4+d5+d6+d7+d8+d9+d10+d11+d12

Thanks for the solution. I read the box as "Optional" and never saw a notice that default values had to be applied.

Yes the "Default Values" property of the "Spreadsheet" widget is optional but needs to be added if you need to display or add some calculation formula.
