I've checked your form and the settings seems good.
I've also checked your form's email history and I see there are notifications and autoresponders that were send successfully by our servers. You can check it in your end by following this guide: http://www.jotform.com/help/293-How-to-view-all-your-form-Email-History.
I do notice that the recipient email addresses are under @ridgeap.com. It's possible that your email service provider is blocking incoming emails from our JotMail IP addresses and domain names, I would suggest that you contact your email service provider and ask to whitelist this list: http://www.jotform.com/help/145-Whitelisting-JotMails-IP-Addresses.
You can also try testing the form by using your personal emails or those that uses @gmail.com, @yahoo.com, @hotmail.com and the likes. See if you're able to receive notifications on those.
