Form submission cut off and weird title

The form notification I received has a strange title =?UTF-8?B??=<noreply@jotform.com> and also it only gives me half of the information on the form.

As I tried to submit your JotForm, I see that the notification is having proper title and all the information is included. It seems that you have already figured out the issue with the notification and added a new one.

I saw your test submission but had the same title and same issue with some of content missing?

For testing purposes, I have cloned your form, after testing, this is the submission I had:

If you look at the form there is more information about "If yes, please provide name of charity or cause and % amount (maximum 50%) that is not visible.

I've tried submitting your JotForm once again by filling all the fields and seems to be that the notifier is showing all the fields correctly.

Since the fields are not made mandatory on the form, there is a chance of leaving the fields unfilled. You may mark these fields as required fields so that the form cannot be submitted leaving them empty.
