After filling out the form and submitting it, I get a page that says the form is unavailable. No response is recorded.
I am using Google Drive integration to upload files.

I have checked your form and I noticed that you have set Expiration Date inside of your form.
Considering that date you have sat as Expiration Date is passed your form is disabled.
If you want to enable your form you need to change Expiration Date or remove it.
