We use JotForm for online intake forms. It works awesome, but we have two distinct issues that recur frequently. 1) Forms are submitted successfully (we have patients fill them out on our computers in the office) but once they are submitted, they never make it to our "Submissions" folder and 2) successfully submitted forms disappear from our "Submissions" folder. Where are they going? How do we find them? Is there a way to stop this from happening?

I am not sure if I have understood your question correctly.
I did check your form and I did not find any integration to send submissions to any folder (googleDrive/dropbox etc). Can you please explain from which folder the submissions are disappearing?
I did send a test submission using your form and the data seems to have been captured correctly. It does gets displayed in your form's submission page. Please check the screenshot below:
