Why do some respondents answers not come through?

In our The Collective {of Us} Feedback form, only three of the respondents answers are coming through. There are 6 questions total and they are all telling us they have completed all of the questions. Why would this happen and is there anything we can do to pull what they have submitted?

We tried making all of the questions required and changing them to not required and that didn't really help.  

Also, responses are showing up for the wrong questions.
