    Hi,

    If you would just like to add a link to the form in your page, you can add this code:

    <a target="_blank" href="http://form.jotform.us/form/51524457264153">Visible link text</a>

    You can replace the Visible link text with whatever text you want for the link to show.  You can also replace the URL with other forms.

    If you would like to embed the form itself to your webpage, here is our guide on how to get the code to do so:

    In most cases I recommend using the iFrame code:
