    I'm not quite sure if I understand your question correctly.
    Can you please provide more details about it?

    Do you want to add some fields in your form and arrange them in the way shown on your screenshot? If so, you could use some of your widgets to do this, for example, the spreadsheet widget that you can find here: http://widgets.jotform.com/widget/spreadsheet
