My text labels for my form are not showing in IE

The labels for the form I am using do not show in IE, but show as expected in Firefox and Chrome. 

Please assist with the coding that will fix this display issue.

Can you please try embedding your form with the iFrame code instead the current one you have? Because, the form itself shows the inner labels of your fields:

When you get your iFrame code, before embedding it in your site, paste it in a notepad, you will notice two parts, the iframe and an script part, only embed the iframe part in your site.
