My form is entirely blank! Please help!

    I planned to use JotForm for my photography contract and I just finished making it perfect a few days ago and now the form is blank. There are no fields, no data, no information whatsoever. Please help!

    Thanks for contacting us.

     

    I checked your form Tamar E. Wallace Photography Contract and noticed that the form fields were all deleted. I checked the form's revision history and found out that the fields were removed recently. You can check this article to know how to view and revert/restore your revision history.
