When people bring up the form, which contains a number of page breaks, often the first page will display a "back" and "next" button and then hang up. The only way most people have been able to get around the issue is to do one or two page refresh requests. I have check and rechecked everything I can think of but can see no reason why this happens.
I have the form in a web page, but I've see this occur whether in the page or when going directly to the form link.

I've checked your form and I saw the problem you're mentioning only on your website. The standalone version of your form works correctly.
