My form has been deleted

    My form has been erased and, moreover, I can not have more access to answers that were filled before. Please solve this problem!

    Thanks for contacting us.

     

    I checked your form and noticed that the form is blank. I checked the form's Revision History and found out that the fields were removed recently.
