    Forms missing - blank pages instead

    After multiple and tedious testing, I deployed a campaign wide survey last night only to have my clients and up on a blank form. What happened? Looks like form fields have been hidden and possible that conditions have been removed.

    Also appears that the history from which I am supposed to be able to restore versions has been removed...

    Hello Swoodruff2002.

    We are very sorry for all the inconvenience.

    I have logged into your account to check the reported issue, and I can see that your first form on top, named Active Week [...], has been disabled. You can set it back to Enabled by clicking on the form > More > Enable, as shown in the image below with numbers 1-3.

    Forms that are grayed out on your My Forms page are Disabled in this manner.

    If your forms appear wrongly, or if there has been an accidental edit done on the form itself, you can also revert back to a prior, known working revision / version of your form, by using our Form Revision History feature - marked in the image with number 4.

    Please first perform steps 1 to 3 to re-enable your form, and see it that helps, before using Revision History. Please let us know how it goes. Thank you.

    I have disabled "Active Weak" intentionally.

    The forms in question are http://www.jotform.com//?formID=51536792116154 and http://www.jotform.com//?formID=51539019535154

    As far as restoring forms from archive, I tried that and my archive has also been cleared. 


    I will need you to restore my account from the server level...

    Thanks - this is the second time this has happened.  Unfortunately, this time I had sent the form invitation to my clients... and so I look like an idiot. 

    I was able to restore a version, but the version I restored has not CSS formatting at all, even though it shows up in the Designer.  I mean, it has style in the designer and when I preview the form, it has not style. 

    I have the forms disabled presently, because I have sent the form link to hundreds of clients...

    The forms #51536792116154 and #51539019535154 have been re-saved in Form Designer to restore the form styles. 

    Please re-enable the forms to see if there is still anything wrong. Thank you.
