When I click to open my form, all of the information is gone! Additionally, the link no longer leads to where it needs to go.
Please help asap
I just had the same issue! Just posted my question too. Very annoyed and disappointed in this. Doesn't give me much confidence in this site's reliability.

I have been unable to replicate the issue, as your form seems to be loading fine for me, please see the screenshot below.
If you are still having the problems seeing your form, can you please let us know the URL to the page where the form is embedded (if it is embedded anywhere). Please also try clearing your browser cache, and try loading your form afterwards. I have cleared your form cache, just in case.
Please let us know if the issue persists.
I restored my form by going to an earlier saved version in my portal. But, I'm concerned that the latest version of my form (prior to restoring an earlier saved version) just disappeared. Is this a bug with JotForm? Could this happen again? Could this happen when I open registration for our school? These are all circumstances under which I am nervous.
Please let me forward this issue to our developers, so that they can check what caused your form to disappear.
I completely understand your concern, and please note that it is not normal for us to lose your form fields or revisions. That is why our developers will be taking a look into it.

I'm in the same boat here... VERY concerned about the future of JotForm. I love this program and the flexibility, but honestly, the reliability factor is HUGE. I sent out a survey last night to my Client A list, and when they went to fill the form, POOF! nothing there. I could loose clients over this, thousands of dollars.
This is the second time this has happened to me after hours and hours of custom form development...
Mine is a free account, and I would gladly pay for the service if I knew it was reliable.
I am not confident that JotForm will be the right tool for me. If there are server level deployments that effect user accounts, we need warnings and we need server level backups.
I'm still waiting to hear how this can be remedied, and I really don't want to hear "Clear you browser", "refresh your cache", or "it's working fine when we look at your form."
I also restored my form from the history, but I need to understand how/why this happened. In a production environment for our business this is completely unacceptable.
I have tried to restore from history, and the version that is out there has been stripped of all CSS. The theme and custom CSS is gone.... From now on, I'll be copying all CSS to a local text editor so I can restore myself if needed...

The versions available for me to restore are with out the custom CSS styling I spent hours creating.
Do you have copies of the CSS of the form prior to the failure last night? The last time I had a good working, formatted version of the forms was Monday 6/8/2015 at 11:30 AM PST.
When I restore from that time, my forms have default theme formatting. It's hours of work for me to rebuild, and my client has lost faith. Can you reset the CSS from your archives?
Please advise... Aldo, what has been done to prevent this from happening again in live production?
I know this just happened to everyone and some are already resolved. I have yet to get a response and I'm getting nervous because we are dead in the water without this form and we open in less than 45 minutes and it looks like this is the one thread that is getting all the attention so i'm asking about mine here. If i'm not mistaken, I was the first questioned posted about this issue today and yet to get a response as I now add to this from edit mode. My thread concerning my form. Squeek squeek

I'm sorry for the trouble caused to you.
We did have some glitch with our form builder which actually caused few forms to appear blank. This issue is already resolved by our back end team. I did check your form and it seems to display all form fields. Your form is now working fine.
I would suggest you to test your form again and get back to us if the issue persists.
