The versions available for me to restore are with out the custom CSS styling I spent hours creating.
Do you have copies of the CSS of the form prior to the failure last night? The last time I had a good working, formatted version of the forms was Monday 6/8/2015 at 11:30 AM PST.
When I restore from that time, my forms have default theme formatting. It's hours of work for me to rebuild, and my client has lost faith. Can you reset the CSS from your archives?
Please advise... Aldo, what has been done to prevent this from happening again in live production?

Thanks! I see that the forms have been restored... and I can in fact access them in the designer... but the CSS is still stripped. Can it be restored too? It was hours of custom CSS and Injections which was really hard for me because I'm just learning.
Also, what I am seeing in the Designer is not what shows when I preview the form, and yes, I have cleared my cache and browser history.
Note the company logo it's showing is the default for the theme employed... which makes me think all theme defaults have been restored.
All fonts should be Tensor Sans. Font point size 14, white. Matrix tables all transparent with white text and standard radio buttons and even column width. Scales should have white text.
And Form conditions hidden fields rules etc... oh my... I hope you can find a version with all this from yesterday AM, last good version was at 6/8/2015 @ 11:34 AM PST.

Thanks for the attention David. I fear my client relationship may be severely affected. I only hope we can restore it to where it needs to be today as I have hundreds of E-mail invitations for people to visit the form link floating out there...
Kind of a race against the clock.

Bugs like this are extremely rare. This is the first time I have seen something such as this that actually altered users forms. We hope to get everything resolved as quickly as possible. Since there are currently quite a few requests, our developers may unfortunately need a bit to get to yours. These are our top priority right now so hopefully it will not be long.

Your issue has been assigned to a developer and been given a high priority. I can confirm that a developer will be looking into your issue as soon as they can. We will update you here as soon as we have any new information. Again, I am very sorry this has occurred and this is quite out of the ordinary.

OK - I have restored most of the CSS and customization while waiting for developer assistance. There are a few issues outstanding. Specifically , I'm trying to get back to transparent background in Matrix tables with white borders and text.
Also, wanting white text on scale labels.
I've pasted the code from previous help sessions into CSS Injector, but deaon't seem to be updating the preview or the builder screens.
I've save recently for back up and copied the CSS text to a local text file for back up. Can you have someone look at the Matrix table formatting (s/b all transparent with white boarders and text, equalt distribution of column widths... and the scale ratings font colors (s/b white).
I'd sure appreciate it as I need to re-broadcast the survey invitation this evening for early morning use with clients.

I've checked one of your forms with the matrix table on it. I see there are a couple of CSS errors on it. I've cloned your form and edited the CSS. Here's the cloned form: http://form.jotformpro.com/form/51601674031950?. You can clone it and check the custom CSS code that I have there. You can use that instead, you can copy my CSS and paste it in your form.

Thanks Charlie!Can you make the Matrix columns even width?
Also, I forgot to mention that somehow I'v messed up the radio button CHECK State. It has a blue dot that is not centered, on top of the default black dot.
I sure appreciate your help. I am slowly but surely learning the CSS through this process.
