My form has been disabled - please help!

    My form ( has been disabled. This is a time sensitive issue, so a quick resolution would be much appreciated.

    I took a look at your account and found it was suspended because you have a form on your account asking for credit card information which is and has not been allowed. 

    We consider credit card and other related information to be "sensitive information" and your form currently is considered to be phishing which is a violation of our Terms of Usage as we do not allow JotForm fields to be used in that way.

    In order to collect payment information properly and continue using your JotForm you will need to remove those fields and replace it with a approved  payment integration. If you are willing to comply then we can gladly reactivate your account but it is recommended to do this as soon as possible to avoid resuspension.

    Thanks for the heads up! We didn't realize that was against the rules. We've put in a Paypal block instead. Can you please un-disable the form?
