I'm having an issue where when I click on the Saved Conditions button for this form it won't display the Saved Conditions that I've set up:

Apologies for the inconvenience. I've cloned your form and was able to replicate the problem.
I'll forward this to our developers to have it checked. We'll update you as soon as the problem has been fixed.
Could you also try reverting back to an earlier version of your form, see if that temporarily fixes your form. Here's a guide: http://www.jotform.com/help/294-How-to-View-Form-Revision-History
