I have another issue thou.
Once I uploaded the form into JotForm. Then I downloaded it into my computer.
When I fill it out, the submission is collected into Jotform.
When I look at the submission or I want to print or save it, the design is totally different.
Is there a way to see the submission or print the submission with the same design as the pdf has?
Attached please see how different the jotform and the pdf looks like.

UPDATE: We are excited to announce that we have just released the new version of JotForm PDF Editor.
Unfortunately the layout of PDF submission is always like that and it can not be customized.
But you can print your form with responses. Inside of this thread you can find out how to do it:

I am sorry but there was nothing in the link that you send me. Could you please resend me how I can print the pdf form filled out? That is what I was looking for.

For me it does not show Print form or submit.
Althought I have the Print Form command in the PDF form.
My email address under Jotform is hrteam@cafeservices.com. If you can log in, please see what can be done.

I also added the Submit button and print button into the form under Jotform but when I end up going to print it it looks like this

When you imported this form http://www.jotform.us/form/51665813371154 from Adobe did you select option PDF Only?
When you imported PDF forms they can not be edited inside of JotForm builder.
They will look as they look now for you and they can not be changed. Also your PDF submission will look same. You can import Form to Jotform, not PDF and then create Fillable PDF form your imported form inside of Jotform.

Would you please then help me what should I do.
If I import PDF form into JotForm and the form can not be changed, how am I going to collect the data into JofForm? Because Submit button and Print button was created in Adobe and therefore the collection of data would not go to JotForm.
I can not import the adobe form as a form because its not a form, its pdf with fallible cells.
How should I collect the data of this form and keep the design as it is even when I am printing it.

Unfortunately imported PDF forms can not be edited inside of JotForm builder.
Option is to create an online version of your PDF form (manually) and then use the fillable PDF generator to create the updated PDF file.
Also you might consider cloning your PDF form:

Then, you will be able to manage cloned version of the form as a standard web form. And then you can create Fillable PDF form form your cloned form.
