After hitting the submit button the form just loop "please wait"

However, this form has two take photo widgets and the first one seems not configured properly. I would suggest to remove the other one if it has no use. You may also want to re-add the other widget to update it.

By the way, would you mind sharing the exact page where you have embedded the form? I can't seem to find it on your page. If the form was embedded, try using an iframe embed code. This should also help. http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
