Simple styled form with the ability to upload multiple video files

    I need a simple yet stylized form the that i can ask for information and also have the ability to upload multiple video files.

    Hi Jeremy,

    You have this form: http://www.jotform.us/form/51136853065151 based on your screenshot. The form is created from our Theme Store. Do you have a specific style or design in mind? Can you please let us know of that?

    Note that you can always edit the form and add fields to it. You can also override the styles, just open the form designer.

    Multi-Upload is also enabled in the form. You can add the video extension in the settings. Here are the steps: http://www.jotform.com/help/29-Upload-Field-Allowed-File-Types. But note that you should consider the file upload limit as well as your account's File Storage Limit. Please check this guide for more information: http://www.jotform.com/help/33-Changing-the-Upload-Size-Limit.

    Lastly, I have modified your field labels to uniform them to left.
