I notice that the inks for the last campaign I launched for a client, are using the jotformPRO Url... Perhaps that's why my video embed forms aren't showing up in the list?
Thanks - concerned that live surveys are being wasted.

Hi Steve,
Actually, the forms are in your account. They are up and working fine. What happened is that your default My Forms view is set to the folder JFA. As you could see, it is highlighted:
You have to click "My Forms" tab in order to view your entire forms:
Click My Forms tab and wait for about 1 minute before reloading the page so it will stay there. If that is not working, then please do the following:
a. Logout from your account
b. Delete your cookies for Jotform. Here's you can do this in Google Chrome: http://www.tech-recipes.com/rx/3016/google_chrome_how_to_delete_individual_cookies/. And here's for Firefox: https://support.mozilla.org/en-US/kb/delete-cookies-remove-info-websites-stored
c. Clear your browser's cache.
d. Login again and My Forms should now be your default view.

As for the Form Domain (jotformpro.com). It is only applicable for paid accounts. We will automatically use jotformpro.com for forms that are created after the account is upgraded.
