Submit Button Disappears

I can build my form and then embed it into my website but every time I refresh the submit button disappears.   Please help?

I checked your page and no matter how many times I refreshed it, the submit button still appeared and was usable. May I ask which web browser you are using that this happens?  As far as I can tell, there are no issues with your embedded form.
