How do I set up the subject line to reflect fields from a form that has 4 different page break submission fields.
If a user submits a form from one of the different options within the form, the fields do not populate int he subject line. Driver code, Fleet, Subdivision.

I have clone and tested your form and I was not able to reproduce mentioned problem. The values of the fields that you have set inside of your Subject inside of Email Notification were shown inside of my Email that I received when I submitted my cloned form.
I see that you are using Outlook for your email. Can you test some other Email client to see if the values of the fields will be visible there? Maybe the problem is related to Outlook.
