NeilVicenteAnswered on November 28, 2011 at 03:27 PMSorry but we cannot and will not create any kind of log in mechanism for a form. This is to prevent abuse from people who take advantage of our service to do illegal activities such as phishing and hacking.
Jotform is not meant for creating log in/sign in forms.
