User Contributed Dropdown is not being displayed in form.

Hello, the App/Widget called User Contributed Dropdown  that you can Dynamically add Options to Your Dropdown Field has stopped working this morning.

Has some changes had a knock on effect or something ?

Can someone have a look to see what the problem is please and let us know what is happening as I have this on 2 forms that are used all through the day

As you can see the app dropdown is missing, albeit it is still as such on the form ?

I did check your last edited form and I am unable to replicate the issue you are having. The "User Contributed Dropdown" widget is being displayed correctly in the form. Please check the screenshot below:

Can you please test your form again and get back to us if the issue persists?

Have you embedded your form in your web page where the widget is not being displayed? Please share the web page URL and we will take a look.
