My form is gone

My form called puppies foster application has disappeared. It is no longer on my website or yours. The only thing left is the theme picture. It was working very well.

We recently had a bug that stripped fields from users forms, it appears as though your form was one of the ones affected.  My apologies.  I restored your form to the most recent working state and it should now be fully available once again. 

If you come across any other forms like this, you can restore them yourself using your revision history:
