Creating a Form with Multiple Possible Answers

    Hello, would I be able to create a form like the example attached? 

    For example the Participant would be able to select M or F for Gender - so there would be multiple choice options in say one column. The same thing for Race/Ethnicity; the participant would be able to choose: African American (=1) American Indian or Alaskan Native (=2) Asian American (=3) Caucasion (Non-Hispanic) (=4) Hispanic (=5) Other (=6) Decline (=0). And so on with Age Range and Primary Disability Diagnosis.

    Wow, thanks for your excellent support. Hmm, I think that gets us pretty close if not exactly. I have created a free account and will do some testing, and will reply back with any questions.
