Can the form output be the same as the original form with values selected?

I pretty much want the output layout to be exactly the same and not just values dumped on a paged.  So if there are check boxes then the layout of the check boxes will be on the output with the correct check box being marked.

If I understood you correctly, you are looking to save the form as it is with all of the information added / selected by the user/client that had filled the form out.

If so I would suggest taking a look at this guide: How to print a form exactly as it is when viewed, but with the user entered data?

You can then print it, save as PDF, etc.
