Some forms seem to be missing page breaks. How to add them?

    I am still having some formatting issues from the transfer:

    The Spiritual Gifts Inventory form:

    On the top of each page should say: Please check the radio button that indicates your personal rating. Remember, 5 = Highly Characteristic; 4 = Mostly; 3 = Frequently; 2 = Occasionally; 1 = Seldom; 0 = Never

    This formatting repeats itself in the middle pages 3, 4, 5, etc  of most of the rest of the pages of my survey. I am looking for it to to only be on the top of each page, with the questions below it.

    I presume that the mentioned form is this one: http://www.jotform.co/form/51605741272855, but I do see the same issue shown on other forms as well.

    This is however very simple to fix, all you have to do is to grab the Page Break tool and bring it up to the position on your form where you need it.

    The Page Break tool can be found under Survey Tools:

    This same action should be repeated for all of the forms that have the same issue.
