Lost data when back button hit

Are you referring to the back button of your multipage form? I'm currently checking the form titled "Safety Plan". I'm testing it on Chrome, Mozilla Firefox latest versions, and Safari 7, and I'm unable to replicate the problem. The data or information filled in on other pages stayed when I click the next and back buttons repeatedly.

May we know if I am testing the correct form and what browser you are experiencing the problem?

I also noticed that you set the Auto Fill Feature, it might be tampering or prepopulating the current session that you have which causes the lost of data. If you want your users to save and continue the progress on the form, you might want to consider using this method instead: http://www.jotform.com/features/save-and-continue/?. I highly recommend using that instead.
