We are experiencing problems with this form "plant-perfection-order-sheet-062415-6a" and unable to determine the source of the problems as this form appears to be configured properly. It works offline and at AFC.
The field names are not correct for many of the menu items. They should all be of the format "Quantity-3", "Option-3", "Total-3" yet that is not what is being shown in the submission report. The menu item field names are not appearing in tab order of the form. When a test order is placed, none of the menu items fields is being received, only the personal information in the top section.

To my understanding you are mainly saying that your Imported PDF Form here http://www.jotform.us/form/51763787072161 is not displaying the quantity correctly.
we do apologize about any inconveniences that this caused as it currently appears that the pdf importer didn't import your form correctly.
Did you remove any of the field items? Or is this pretty much how it was imported already?
Looking more into this issue I found that even though your field name format is incorrect it's possible to manually correct that though I know that is a bit of a hassle to have to do.
Have you tried deleting it and re-importing it to see if it will import properly?
There is more than one issue. The field names are not correct. The field options are not correct. On the pdf form, data entered into the fields is not being received on form submission.
I have imported this form several times and the same problems persist.
Could you confirm which one are you having problems? In your first post, you mentioned the PDF form "plant-perfection-order-sheet-062415-6a". The examples you mentioned, however, seems be to related to the web form http://www.jotform.us/form/51763787072161.

Unfortunately, I can't seem to find the field names "Total-3" and the others with the same format in the PDF form. Could you give us more information which form you are having problems, and if it's possible, a screenshot of what you are seeing so that we can better visualize the issues?

The pdf in your screenshot is correct. There is not a problem with the form itself. It functions properly. The problem is with submissions. Here is a report for a test submission: http://www.jotform.com/table/51765881835064
Please note the incorrect field names and that the data is incomplete. Fields are empty that had data included in the test submission
