I see that I am able to integrate JotForm with MadMimi, and can make all form registrants in one MadMimi list. I am wondering if I can do conditional integration. Specifically, I have a MadMimi list for all families with 3rd graders. If someone signs up a 3rd grader for a program (program is open to multiple ages), can JotForm conditionally add that family to the 3rd grade list? I know that is getting complicated.

Unfortunately, this option is not added in the integration, since you can only integrate a form to a Mad Mimi list at a time.
However, here is a possible workaround:
1) you could create and integrate a form each list.
2) Grab the direct link of each form:
3) Then create another form with the options lists:

Create conditional logic to redirect to a form depending on the answer, as explained on this guide: https://www.jotform.com/help/317-Configuring-Change-Thank-You-URL-after-submission-condition, in this case the URL to enter will be the direct link of one of the forms.
So, depending on the option of the user, that's how the will sing up to a specific list:
