All my forms have been disabled?
can you advise why and reinstate ?

Your account and forms was suspended by the phishing filter because of the suspected phishing forms you have created (Account Access Form).
Tell us if this is not the purpose of your forms by providing us more details. If you can share to us also the URL of your website where you have used the form, we will check further and see if there is any reconsideration we can do.

The problem started when I added the following button to this form - see below

This opens a google sheet that contains the data of boards that have been put up by various agents
I used this method from your Forum as suggested by Jotform Team so I hope this can be resolved urgently.

The form that our system suspended your account for was your "Account Access" form. We do not allow for the collection of login details such as usernames, passwords or account keys. I have reactivated your account but I was strongly suggest you remove that form. It does not appear to be used for phishing purposes, however, our system will interpret it as such.
Make sure you purge the form from your trash as well:
The process to purge the form is the same as restoring it, just click on purge instead.
