Have not heard back from anybody about this. My third attempt if this cannot be resolved we will need to look for another solution.
I have two other forms that need the same fix.
I am unable to set up notifications / email for these forms:
(this one is most urgent, federal requirement)

I believe you have imported a PDF form directly to JotForm from FormsCentral. You can only change the recipient of the Email Notification sent when someone submits your form by changing your JotForm's account default email address.
