I would like to upgrade, but my form is no longer under my forms!

The form is actually present in your account. I think your current session have expired so I would suggest logging out from your account if the system didn't automatically do it for you. Login again and you should see the forms back.
I have also reactivated your account, set your status to Active from Overlimit.
