How do I create a new survey using an old JotForm account
I am the new ED for CityMusic Columbus. We have a JotForm that appears on our home screen, and I want to change it. I am not sure how to do it. Our JotForm account uses an old email address that is no longer in service, can you help?Page URL:

The 'Welcome to CityMusic Columbus' form seems to be already moved to the account trash folder.
The 'citymusic' account is currently associated with the next email address - email@example.com.
