Created a form as a guest.Now have account an need it there

Hi,

I created a form as quest and now have account with you but the for could not be imported as it says unauthorize and also give error on it url to be deleted form the form ID is 51826784208866 . Kindly help restore the fore and guide how to attach it to my account. It is urgent thanks!
