Is there any way they can tell you who submitted the form when someone submits a threat, threatening to come to where you live, presumably to hurt or kill you, and has filled out the form providing informationt hat shows that they do in fact know who I am and where I live?

You can see the IP address of the user on the submissions page. Click on the Settings icon and then select the "IP" option.
Then print out the whole page and take it to Police. If you can provide them the IP address and the time, they can find out the person who threatened you from the ISP.

I don't see a submissions page. All I see is how to edit or delete my form.
