How can you tie the jotform back to a Salesforce campaign?

I'm not sure I understand your concern exactly. But if you mean to say, how to integrate JotForm with Salesforce, you can follow the steps provided in this guide: Flexible-Data-Integration-with-Salesforce

I've completed the integration of JotForm with Salesforce. What is my next step in ensuring that the JotForm is recorded in Salesforce under each contact's name? I saw an example in your guide that shows a Campaign ID number. Does that tie the JotForm to the contacts in Salesforce? Is there something I need to do in Salesforce to send the JotForm to my contacts? Am I sending them the link to the survey as part of an email campaign?

Since you have integrated your form with Salesforce, that's good to go. Also, as long as you have mapped your form fields to the fields on your Salesforce database accordingly, you should not have any issues. Our smart mapping will match your form fields with its equivalent to your Salesforce. The Campaign_ID shown on the example in our guide is just an example on how to use text boxes for custom fields.
