Please recreate your Autoresponder (my walkthrough note and Walkthrough note) in order to fix your Mail Form Field Codes. Some form fields aren't presented in your Autoresponder that's why some informations aren't included on submissions through Email.

After that, you have to create new Autoresponder, so it includes all form fields. That should fixed your problem.
