I want to create a form in which the respondents can upload his/her photo, as shown in my attachment here. Can the Jotfom enable me to do so?
If so, how can I access the photo and all the answers submitted?

Yes, you can easily add a File Upload field to your form, as depicted in this guide: How-to-create-upload-forms.
If you wish to limit the file types that your users can upload (such as JPG and PNG), you can edit the allowed file extensions by following this guide: Upload-Field-Allowed-File-Types.
You can also limit the allowed file size by Changing-the-Upload-Size-Limit.

Great, so how can I access the photos and all the answers submitted?
From what I can gather from your website, as long as I have set the Notification to be via email, I should be able to get notified every time when someone fills in a form. But I just want to ascertain this one more time, if there are also photos uploaded in the form (as shown in my attachment shown earlier), will I also be able to receive their photo?

Yes. The uploaded files/photos should also be included in the submission email sent.

You can also view the submission data, including uploads in your form's View Submission page.
