I am signed up through the email firstname.lastname@example.org and i'm not quite sure what my Username is. I have my password written down and saved but apparently even when i try and submit a form to reset my password as an attempt to retrieve my username the email won't arrive to my inbox.
I am really trying to just edit an existing form I have on my account, please reference the photo below. When I use the jotform on my mobile device the "TITLE" portion of the form is skewed. The fill in portion for that section is floating off onto the right of the screen.
The username for your account is "50PlusReport" and the email address for password reset purposes is "email@example.com".
I checked our mail logs for that address and it does appear as though the password reset was sent successfully. Checked your spam/junk mail boxes to ensure they are not being filtered there. If you are still unable to recover your account, let us know and we will be happy to see what else we can do.

I am still not receiving the reset email. I have checked the spam/junk. Nothing is there.
Since it does appear as though you are the owner of the account, I went ahead and changed the email address associated with it to "email@example.com". The password reset should not be sent there. Let us know if you have further trouble receiving the reset and we will have another look.
