My form has other boxes, etc. that is mysteriously showing up when previewing and saving as a .PDF, please help!!!

    I have checked you account and it does not have any forms listed.

    Could kindly provide the link to the form in question, I have also cleared your account cache, try and regenerate the PDF to see if this has resolved the issue.

    The first form entitled “Academic Program (AP) / Financial Administration Enhancement (FA)……when I view it, it doesn’t show all the headers, under the focus area, there is an extra box named “other” that I DID NOT put there.  There is an extra line under the comments box.  Also, the acknowledgement box doesn’t show the information I typed in it.  Please help!!

    The form I have created, when turned into a fillable form, contains what I am calling visual "artifacts." They are behind and below the fillable areas, duplicating the fillable areas. 

    The text areas that I am using as headings also do not show up, or only partially.

    Is there something about the way I have the form formatted that is causing this to happen?  Please help!

    I'm sorry for the trouble caused to you. 

    I did download the PDF version of your form and I am unable to replicate the issue you are having. PDF form does display correctly without any problem. Please check the screenshot below:
