I have tried your pdf creator and the create pdf within the form properties but neither way will save the forms background or images.

I can confirm this happening on a cloned version of your form. This is how the online form looks:
The generated PDF is missing the image at the top, and is missing labels for input fields where you are using placeholders on the online form:
I have raised these issues to our developers, so that they can take a look at what can be done on our end to possibly fix this and make your PDF form looking more like the online form. We will let you know through this thread if we have any updates for you regarding this issue. Thank you.
