Change form title

    When I share the form from the template 'Oklahoma Standard Poodles' it still has the title 'Oklahoma Standard Poodles'. I have changed both names in preferences, what am I missing? It should say Cat Adoption Application Saudi Arabia. Thanks

    I have inspected your form http://www.jotform.com/51932411680451 and I noticed that you have successfully changed form title and page title.

    I have also found same form in out templates gallery that you used and modified. Where did you share your form?

    I did not found your form in our templates gallery.
