Your "Contact us" page uses frames which are loaded from two separate html files. This is not the most ideal setup to install Wishbox on. In fact, there is no way to load your Wishbox form on the exact html source code you just posted.

Wishbox will not be able to capture both frames (navigation and body) since it can only be installed on a single html file, the most optimal being body_contact_us.html, since it contains the important part of the page sans the navigation frame.

Thank you Neil for your feedback.  I have a contact form on one of the webpages that is working fine.  They fill the form in to request a free estimate.

Here's another page I would like to put the "critique" form.  Here's my HTML code.  Can it go there somewhere?  I am confused by my software having HTML code and "Code".  Not sure where to embed my code.  But since I have one working, surely this second form on the website can work too.

You can embed the codes between the body tags. Please do try then let us know if it works or not. Best if you provide us the URL of the new page in case things go wrong.
