1. I have looked at your solution to conditional fields on in the form but I can not get it to work. I set all the conditions in the top field. THe bottom field does not give me any options other then
"Then after user clicks next button skipt to "Please Select" There are not option in the dropdown box.. SI I can not set the option. What am I missing?
2. I want to create a form that populate another form
SO a user (user1) registers an activity ( a code is created for that activity)
The user gives that code to a person (user2) who will rate their experience resulting from the activity/service user1 provides
User2 signs in with the code and leaves their evaluation of User1. The results populate a spreadsheet. automatically.
IS this doable with Jotforms?

Conditional field: I am not sure if I have understood your question correctly. With which form you are having this issue with? I did check all the three forms you have in your account but none of them has any conditions.
I would suggest you to please explain your requirement a bit more in detail and we will surely help you.
