When our clients try to use our form, they receive an error message when they submit the form, we are receiving the form regardless but obviously it is a bit tiresome receiving phone call after phone call with this problem.
Please can you let me know how we can amend this.

Is this the error message that your clients are getting?
There is no issue in your form since you were able to receive the submissions. The problem lies on the "Thank you page - Custom URL". As you can see once the user click the "Submit" button it will go to the thank you page. You are using a custom URL (http://http://www.holysport.co.uk/) for your thank you page which is down for the moment. You need to contact your web service provider.
However, you can use the "Default Thank You Page" or the "Thank You Message". Please refer to this helpful guide: http://www.jotform.com/help/211-Setting-Up-the-Thank-You-Page
